Vision Service Plans is the Provider on this plan

NMR Vision Plan - This plan is for groups of 5 or more employees.  The NMR Vision Plan has been available since 1992.  The plan is a Multiple Employer Trust arrangement with Vision Service Plans.  We also offer this program through the CBSF Trust, combine with WDS or IHC CBSF program and get one bill.

 

BEST Life & Health - Vision plans stand alone for groups of 5 or more, employer paid and voluntary (voluntary minimum is 5 lives)

 

Madison National Life - Davis Vision Plan. Group's of 2 or more. 

 

Confident - Voluntary Vision Plan - down to 2 lives. In and out of Network options, Competitive Rates.

 

Security Life - Voluntary Vision Plan - Down to 1 live if there are at least 3 eligible employees.  Any provider.

IndemnityPlus Dental Insurance Plans -  These plans are for groups of two or more employees and offer employer paid and voluntary options.  Plans feature Adult & Child Orthodontia, Implants, Optional 2-Year Rate Guarantees and much more.  Discounted dental if you buy their vision too.  Rates and commission/bonus programs are competitive.  

 

Washington Dental Service (WDS) - CBSF Trust Program for groups of 2 or more unrelated. Must be a Chamber Member

 

Madison National Life - NMR Dental - 2+ employees.  Wide variety of plan options, includes Implant coverage.  Voluntary available down to 5 enrolled lives.

Two programs offered, CBSF Trust set plans and rates or direct custom options.

 

Confident - Group and Voluntary dental.  Rates recently reduced.  Partner with Vision plan.
