Includ with your RFP current benefits, current and renewal rates.  Health risk questionaire for the group and census.

 

This plan generates rates assuming a health group.  To get final rates we would need employee enrollments with health questions answered from all enrolling employees.  We can submit a pre-scrreen using other carriers employees health risk forms.  This would give us an idea of the offering but the offer would not be final until the ATA forms are submitted.
