Group Dental
Flexident, underwritten by Madison National Life Insurance Company, Inc. and Standard Security Life Insurance Company of New York, is a fully insured dental suite of insurance products for employer groups with 2 or more employees. Custom plans are available for groups with five or more employees.
Custom plans
For groups with at least five employees, you can customize the group's plan by selecting frequency limits, types of services, deductible, annual maximums, waiting periods, orthodontia and coinsurance.
Multiple plan designs
For groups of two or more, four different plan designs are available with annual maximum benefits ranging from $1,000 to $2,000.
PPO and Indemnity
Flexident offers two different PPO plan arrangements and also a Traditional Indemnity option. Ask a representative for more information.
Employer contribution
The employer can pay a portion, all or none of the employee premium. Rates are partially based on participation and we offer multiple participation tiers.

Group Vision
Group plans for two or more lives are available with multiple frequency and copay options to fit many needs, utilizing the Dental Vision network with nearly 45,000 providers in all 50 states.
Group Vision is underwritten by Madison National Life Insurance Company, Inc. (Group Policy MNL GVIS-POL 1107), and in New York and New Hampshire, underwritten by Standard Security Life Insurance Company of New York (Group Policy SSL GVIS-POL 1107).

Madison Dental for Individuals
Madison Dental is underwritten by Madison National Life Insurance Company, Inc., and offers quality, affordable dental insurance options in all states except New Hampshire and New York. In the states of New Hampshire and New York, Madison Dental is underwritten by Standard Security Life Insurance Company of New York.
PPO and Indemnity
Madison Dental utilizes several dental networks which provide access to more than 81,000 providers nationwide. Or, the Indemnity plan option allows your client to see any dentist without network restrictions.
Optional orthodonia discount
The OrthoCare discount program provides a 15-20 percent discount on most services performed by an OrthoCare orthodontist. The OrthoCare product is not an insurance benefit and is not available in all states.
